<p>An incident where a pebble jammed the deadman’s pedal safety device on a Sydney Millennium train was an isolated event that has not reoccurred, RailCorp said.</p> <p>The incident was in April this year when a small pebble from a driver’s shoes fell into a gap between the pedal and its surround.</p> <p>"That was an isolated incident," a RailCorp spokeswoman said.</p> <p>"EDI [the train’s manufacturer] have since identified a fix for it which involves fitting a mat over the pedal."</p> <p>The matter was a technical, not a safety issue as the train had not actually moved once the pebble had jammed the pedal, she said.</p> <br />
