Deadman’s brake concerns date back to 1994: Waterfall Inquiry

<p>New South Wales senior rail managers were warned about concerns with the deadman’s brake system as early as 1994, the Waterfall Inquiry was told yesterday (Monday, August 11).</p> <p>A 1994 consultant’s report warned of the dangers of an incapacitated driver keeping the deadman’s pedal set, the <em>Sydney Morning Herald</em> reported this morning.</p> <p>The memo said that this system, without the addition of more vigilant controls, represented an intolerable risk, the paper reported.</p> <br />
